Referring Vets

The most expedient way to refer a patient is to complete our referral form and fax it to 603-672-3450 or complete the form below. Any other pertinent medical records and lab data can be faxed. If radiographs are to be sent please indicate how they will be transferred. The referral form can be printed from this site or you can request that printed copies be sent to your office.

We will contact your office when the referral has been received and reviewed. Please make a note in the comments section if you would like to consult on the case prior to patient scheduling. We will make attempts to contact the owner to schedule an appointment, but prefer that you also provide the owner with our number and advise them to call once the referral form has been sent. Directions and a map are available and can be printed from this website.
